On request of a party and for good cause, the court in a proceeding under sections 46b-450 to 46b-553, inclusive, may order the name of the child changed. If the court order changing the child's name varies from the name on the child's birth certificate, the court shall order the Department of Public Health to issue an amended birth certificate.
