(a) Each third-party administrator seeking to renew a license issued pursuant to section 38a-720j shall submit a renewal filing in the form and containing such information as the commissioner prescribes, including evidence that the surety bond required under subdivision (1) of subsection (a) of section 38a-720j and, if applicable, subsection (h) of section 38a-720j, remain in force. The information contained in such renewal filing shall be verified by at least two officers of the third-party administrator.
(b) The renewal filing shall include the complete names and addresses of all insurers or other persons with which the third-party administrator had written agreements during the preceding fiscal year.
