Each homemaker-companion agency, prior to providing homemaker services or companion services, shall (1) provide the person who receives the services, or the authorized representative of such person, with a written notice that the agency provides nonmedical care, and (2) obtain the signature of such person or representative on the written notice. The agency shall maintain a paper or electronic copy of such signed notice until such time that the person who receives the services ceases receiving services from the agency and make such copy available for inspection upon the request of the Commissioner of Consumer Protection.
