(1) If a number or personalized license plate issued under this article becomes lost, stolen, mutilated, or illegible, the person who is entitled thereto shall immediately apply for a substitute. Such application shall include evidence satisfactory to the department that such plate is lost, stolen, mutilated, or illegible and payment of the required fees. If the plate to be replaced is in the possession of the applicant, the plate shall be surrendered to the department along with the application.
(2) If an application made pursuant to subsection (1) of this section is accompanied by the personalized plate to be replaced, the department shall reissue a substitute plate bearing the identical sequential combination of letters and numbers that appears on the original plate.
