(1) In addition to any other method permitted by the commission or office, a person may submit a request for evaluation by mail or online. The office shall develop an online request for evaluation form that is accessible from the commission's public website.
(2) The commission and office shall permit a person to submit a confidential or anonymous request for evaluation.
