(a) The clerk of the court shall collect a fee of fifteen dollars ($15) per signature for any document that is required to be authenticated pursuant to court order.
(b) Each document authenticated by the clerk of the court shall contain the following statement:
“___, Clerk of the Superior Court, County of ___, State of California. Signed pursuant to court order dated ___ in the matter of ___ petitioner v. ___, respondent, Case No. ___.”
