The notification of the number of delegates and alternate delegates shall be in substantially the following form:

Statement of Number of Delegates and Alternate Delegates to Democratic National Convention and of Number of Delegates and Alternate Delegates to Be Selected from __________ Districts

You are hereby notified that the number of delegates and alternate delegates to represent the State of California in the next national convention of the Democratic Party is __________.

You are hereby notified that the number of delegates which may be selected from all __________ districts as a part of the delegation to the national convention of the Democratic Party is as follows:
