Application for registration of securities by qualification shall be made by the issuer of the securities by filing with the commission the following documents:
1. An application for registration of securities by qualification as provided by § 44-1893.
3. A nonrefundable registration fee of one-tenth of one percent of the aggregate offering price of securities to be sold in this state, but the registration fee shall not be less than two hundred nor more than two thousand dollars. The amount by which a registration fee exceeds one thousand five hundred dollars shall be allocated to the office of economic opportunity operations fund established by § 41-5302.
4. A consent to service of process as provided by § 44-1862, if the issuer is not domiciled in this state and is not an entity organized under the laws of this state.
