Arizona Revised Statutes Title 37. Public Lands § 37-1423. Registration; renewal

1. An application for registration on a form prescribed by the state forester and the fee prescribed by § 37-1422.

2. Proof of insurance as required by this article.

3. A copy of an inspection certificate that is issued by an insurer or an inspector with whom the insurer has contracted.

4. A copy of the owner's or operator's business license.
