A. Each applicant for licensure or certification shall file a verified completed application in the form and style required and supplied by the board and accompanied by the appropriate application fee prescribed in § 32-1527. The filing of an application grants the board the authority to obtain information from any licensing board or agency in any state, district, territory or county of the United States or another country, from the Arizona criminal justice information system in the department of public safety and from the federal bureau of investigation. The fingerprints submitted shall be used to obtain a state and federal criminal records check pursuant to § 41-1750 and Public Law 92-544. The department of public safety is authorized to exchange this fingerprint data with the federal bureau of investigation.

F. The board may interview the applicant to determine whether the application is sufficient or whether the applicant otherwise qualifies for licensure or for a certificate.
G. Applications are considered withdrawn on the occurrence of any of the following:
1. The request of the applicant.
2. Failure of the applicant to appear for an interview with the board except for good cause.
3. Failure of the applicant to submit a completed application within one year after the date of the mailing by the board of a statement to the applicant of the deficiencies in the application under subsection E of this section.
4. Failure of the applicant to show, within one year after the interview provided for in subsection F of this section, that the completed application is true and correct.
