The ombudsman shall prepare and distribute to each facility a written consent form which sets forth that in the event of an elderly abuse investigation, the patient, resident or client of the facility consents to the release of the investigative report to the legal guardian or other person named on the consent form. The ombudsman shall not be required to disclose the results of any investigation or furnish a copy of the written report prepared pursuant to subsection b. of section 3 of P.L.1983, c. 43 (C.52:27G-7.2) to any person other than the resident, legal guardian or named person on the consent form.
This written consent form shall be given to every patient, resident or client upon admission to the facility.
