A person transacting business in this state under an assumed business name shall register with the secretary of state, on forms furnished by the secretary of state, an application for registration of the assumed business name, including but not limited to the following information:
(1) the name and business mailing address of the applicant;
(2) the complete proposed assumed business name; and
(3) a description of business transacted under the proposed assumed name.
